Hi Kay.. What name would the ammonia levels be unders.. I always ask for copies of my records.. I have papers upon papers of my records.. Any helpful hints would be greatly appreciated.. Thanks.

Hi Deb, I was in the hospital on 9/9/99...of all dates.

About 5 months ago, i requested my medical papers, they were about

70 pages long. As I was reading them, I found out all my stats.

It showed my ammonia level along with everything else, so just ask for

your medical papers and start looking up the things it says..

Deb... I don`t know that ammonia (NH3) levels are usually put on

reports. The normal ranges vary quite a bit and they are rather

subjective, The BUN may give some idea.

If your ammonia levels were too high you would know it as signs of

hepatic encephalopathy would be seen.

Those who have variceal bleeding would have raised NH3 levels as blood

contains proteins that the liver might not metabolize.

The treatment for high ammonia levels would be Lactulose and a diet

containing little or proteins. I sent a page on LFTs that mentions

ammonia.

Thanks Dawn. I had thought about making it to the SSRI conference. Pessa and

I had talked about it, but I have a week long trip to Wisconsin at the end of

September, then leave for New Hampshire on the 11th of October for another week

and I have a show of my work in Ann Arbor in-between. Just can't take a third

trip or I think my son and husband would lock the door on me when I returned. I

did think how cool it would be to see some of you in person. So many, like you,

Pessa and are heroes to me - bigger than life somehow - incredible

people for what you've been through and what you've tried to do about it. Yeh,

it really would be great to see all of you.

able to share their stories at the SSRI conference. If you can connect with

them, will you encourage them to get in touch with me? I have a plan that

will have quite an impact.

Yes Dawn, I was able to speak with someone at the FDA for over an hour a week or

2 ago. No, he couldn't give me any answers as to why they're protecting the

rights of the drug companies and ignoring the health and safety of users. He

said that I " had some powerful questions " . He's heard a lot of stories about

Paxil and knows all about the problems, yet said that, " Things aren't going to

change anytime soon " . He did say that the judge who ruled that GSK temporarily

stop using their current ads, didn't have the legal authority to do that. He

said that a certain district court would have to do that. Shoot, can't find my

notes about the conversation, but I read that somewhere else too. I'm going to

call them again this week and ask to speak with a bigger honcho.

Deb,

Your nurse is wrong!! I know people who were undetected for over 4

yrs and the virus came back. It's most likely that the virus goes

dormant because of treatment. It will always be somewhere in your

blood.

Any occasional drinking will only cause the virus to hang around and

cause more liver damge at a faster rate.

Billie,

When I was diagnosed with Hyperthyroid, my symptoms were high pulse

rate (120 bpm), trouble sleeping, nervous energy, poor

concentration, no periods, chestwall pain, joint aches...those are

the ones that come to mind...I had a lot. My T4 was 19.4 and my tsh

was <.08. Those were my only tests.

As for posting, that's what you call it when you put a message up on

the board. You do it all the time!!! Now you can laugh at

yourself; we all need a little comic relief, right?

Actually, the throat problem ends up being the OPPOSITE of reflux. It is

physically impossible for me to have acid reflux because the nerves that

tell the LES (lower esophageal sphincter) muscle to relax (open) have been

totally destroyed. Treatment generally consists of either ripping open

the muscle (with a balloon down the throat), or surgically slicing the

muscle (like a jeweller would cut off a too-small ring.)

My girlfriends all hated me when I was pregnant because I never once had

heartburn the entire pregnancy, and they were all buying Tums by the case!

(There's a good side to everything, huh?) ;o)

They're not quite sure exactly what causes achalasia. Some believe it to be a

virus (or many different viruses), some believe it to be an auto-immune

disorder, and some believe it to be a genetic predisposition of some sort.

Here's an excerpt from a website about achalasia:

The specific cause of achalasia is unknown. Normally, the LES contracts and

relaxes in response to opposing signals from two groups of nerve cells located

in the wall of the esophagus. For unknown reasons, in patients with achalasia,

an inflammatory reaction targets nerve cells in the esophagus, particularly

those that signal the LES to relax. This reaction causes these cells to

gradually disappear. The end result is that the LES fails to relax and thereby

creates a blockage for swallowed material to enter the stomach. To make matters

worse, nerve cells in the lower two-thirds of the esophagus are also destroyed.

These cells are needed to make the esophagus contract and propel food toward the

stomach, a process known as peristalsis. Thus, people with achalasia often

accumulate large volumes of food and saliva in the esophagus. Food can only

enter the stomach when enough pressure builds up in the esophagus to overcome

the resistance posed by the non-relaxing LES.

Here's another that gets into the pathology of achalasia:

The predominant neuropathologic process of achalasia involves the loss of

ganglion cells from the wall of the esophagus, starting at the LES and

developing proximally. The degree of ganglion cell loss parallels the disease

duration such that, at 10 years, ganglion cells likely are completely absent. At

the level of the LES, the loss of inhibitory nerves is demonstrated by a lack of

the usual nitric oxide synthase and vasoactive intestinal peptide (VIP)

immunohistochemistry staining in the myenteric nerves, whereas, in the

esophageal body, acetylcholine-containing nerves are affected. Extrinsic nerve

alteration is characterized by Wallerian degeneration of the axoplasm and myelin

sheaths within the vagus nerve and dorsal motor nucleus. Anatomically, the

circular muscle layer at the LES is thickened, but, microscopically, individual

muscle cells are normal.

The physiologic process of achalasia is correlated most directly to the loss of

the inhibitory nerves at the sphincter, resulting in failure of the LES to

completely relax and a hypertensive LES pressure. Elevated LES pressure greater

than 40 mm Hg is observed in more than 60% of patients. The loss of nerves along

the esophageal body causes aperistalsis, leading to stasis of ingested food and

subsequent dilation of the esophagus. Nonperistaltic isolated contractions or

low-amplitude simultaneous contractions of the esophageal body may be observed.

If high-amplitude (>60 mm Hg) simultaneous contractions occur, the entity is

categorized as vigorous achalasia.

There is a parasitic disease called Chagas disease (typically only seen in South

America) that can have symptoms very similar to achalasia, but it's a systemic

disease, and can affect different parts of the body in varying degrees. I

believe Chagas is fatal approx. 30% of the time (in people who have major

infection of the cardiac system.) Fortunately, achalasia is limited to JUST the

esophagus and lower esophageal sphincter, and doesn't " spread " to other parts of

the body.

I'm very interested in the connections between vasodilator compounds and adrenal

hormones in the body and the relationship to swallowing problems or reflux

problems. The more I learn about this, the more I'm amazed at how intertwined

all the body's systems are. I just might have to go back to school and get a

nursing degree after all... between my achalasia and my thyroid/adrenal

experiences, I'll be practically halfway through medical school just in reading

about my own situations!!! LOL
