Many people simply skip their last dose before surgery. The absolute

safest is considered to be 72 hours without LDN prior.

It's a quandry, because you don't know when labor will begin. I had 3 children without pain meds, and applaud you for your desire to do this. For a first child your labor will likely be 8 hrs or more. This should give LDN enough time to clear before you might need pain meds. Pain meds will work about 6 to 8 hours after your last dose.Fentanyl is effective in controlling pain, for people who take LDN. This is a short acting IV drug, that is commonly used for anesthesia.
