If the individual's immune system is weakened by the bout with pneumonia, and they succumb to something else afterwards, the fatality is likely not attributed to the pneumonia. Or if they've got pneumonia but are felled by what is apparently a heart attack, which one gets the blame? I don't recall the exact quote, but it was to the effect that in the end, all deaths are due to heart failure.
