You can enroll in the CMSC/NARCOMS Multiple Sclerosis Patient Registry by submitting an enrollment questionnaire. The enrollment is free of cost and promotes Multiple Sclerosis research. Your information is kept strictly confidential.

You can enroll now in the Multiple Sclerosis Patient Registry now using our online system. We use the latest security measures to ensure confidentiality.

You can submit a request for a questionnaire to be delivered to you by mail.
