LDN does not work with PPMS very well at all but it can help a little when first taken. It is difficult to find a dose level that does not make muscle stiffness and spasms worse. In my experience a dose of just 1.5mg is best for PPMS. LDN is worth trying but don't expect any miracles. Steve
