Once it forms a compound with IgE, how is it eliminated or metabolized for example? Given this compound cannot bind to mast cells, does you body then produce way more in the form of a positive feedback mechanism in order to compensate??? Can you discuss briefly the reasoning behind why we cannot re-measure IgE after you begin dosing? (He said Genentech has been able to do it at their lab, but they can't yet produce a test that could be replicated outside of really expensive lab conditions....) Guest guest Posted October 13, 2005 Report Share Posted October 13, 2005 When IgE is bound it is ineffective. No matter how many allergens invade the body that particular IgE can not cause a problem. We have many (hundreds, thousands) of IgE. IgE keep being manufactured by the body. When an IgE is bound it can live for up to a year until it gets routed thru the liver, broken down & excreted. The commercial tests for IgE only count IgE, whether they are bound with Xolair or free floating. Genentech & Novartis came up with a test that can measure bound IgE. It is VERY expensive and not available commercially. SO, the physician can order another IgE level but it will not be a true accounting of how many free floating IgE are still in the system. If he is silly enough to order the test, he may be silly enough to change therapy because of the result. If any of your physicians want to change your Xolair therapy because of a re-evaluation of the IgE, PLEASE tell them to call Genentech and speak to one of the researchers. Don't call them stupid to their face as this might ruin your Dr, Patient relationship, gently redirect them. I have had several physicians call & say " I'm taking my pt. off Xolair because their IgE keeps rising " . After I educate them, they agree to let their patients to keep using Xolair.
