Last week I sent a fax to the PA-C requesting a copy of the paperwork listing the surrendered meds. On Monday I got a call from Med Records and they said I needed to fill out a release form so I asked that they mail it to me. I rec'd the form yesterday. I filled it out today and it's ready for tomorrow's post. In the meanwhile I forgot to fax the grievance coordinator my phone bill copies, showing how many times I called their triage line vs. how many times they called me back (including the clinic's " timely fashion " )! Tonight I typed a cover letter with the faxed bills, apologizing for the delay, and then said that I'd reread the original contract but somehow missed the part that says you must surrender your medication upon contract termination in order to be permitted a rereferral, if necessary. I circled the section that said you had to bring your meds with you in original bottles to each visit to show that I was aware of my responsibilities. So I politely asked if the coordinator would be kind enough to show me where that med surrender clause is located and if it's on a separate sheet, to please mail me a signed copy because that would mean that I am missing paperwork for my own files. Remember, I was told that I HAD to surrender the meds 1) if I would ever need to be rereferred back and 2) for proper disposal because I would be violating EPA laws by dumping unused meds down the toilet. This made sense to me because my was-husband #1 is diabetic and we always kept a sharps container for his used syringes that I would turn in to my Dr's ofc or the hospital for incineration. I just opened today's mail about an hour ago (11pm). In it was a letter from the clinic's nurse manager... " This is a copy of your record that you requested on 11/13/08. Thank you for contacting us so we can continue to serve your needs " . Attached was their handwritten copy of my medications instead of the computer print out that I gave them at every visit. At the end of the list are the notes from 11/7/08: " No further narcotics - pt may be re-referred by Dr C after surgeries. Pt relinquished - (insert itemized counts of med list, which coincidentally, I do have 1 discrepancy with) - flushed down commode " . SAY WHAT????!!!!!!!! " flushed down commode " ?????!!!!!! Can't wait to hear the grievance coordinator's response to this one when my next fax will be for her to explain how it is legal for them to dump but not me; and if it's not illegal for me, then why did they take my meds; why is the medical record clerk insisting on a release form yet the ofc mgr sent me the notes; or perhaps do I need to address this entire issue with the EPA and/or DEA for a detailed explanation as to who's in violation of which laws since they don't seem to be consistent in their practices??!!!!! I'll sleep well tonight...for the first time since " the ordeal " ! Nighty-night everyone.
