Well maybe if you had a CERVICAL fusion...is that what you had? The main goal of PT for a person with a bad back is to strengthen the core. Doing that yakes an incredible amount of srtain off the spine and can greatly reduce pain. Also, if you have a bad back and esp with a lumbar fusion, your sacrum and hips have problems, and you may not even know that something is off in your hips and causing back pain. Everything is connected. With a cervical fusion, I still feel that you can strengthen your neck muscles...what you need is more hands on therapy which includes cranio-sacral and myo-fascial release techniques...some of which you can learn to do at home. Many PT's do that, but as I said in an earlier post, if you find a PT who is also an MT...you can have massage and all those above listed therapies for cost of your co-pay. Guest guest Posted May 29, 2009 Report Share Posted May 29, 2009 I have been lucky with my PT--I have a herniated disc and cervical stenosis, as well good old DDD marching through everything. I dropped the first therapist I had since I sensed they weren't going to help me--I really don't think she believed I had pain. I managed to hook up with a top-notch PT school, being part of their clinic. I really love the direction the new therapist has been going, with building my core, my legs and arms, and learning how not to let my neck muscles " guard " . I think I am overall the strongest and healthiest I have been in my entire life and I have actually been able to reduce my medications. I do spend 30-45 minutes 6 days a week doing the exercises he has showed me, on top of 30 minutes of cadio they are having me do 5 days a week. He also has been able to do pressure point massage, which finally got some of those muscles to relax. Combined the PT with a good accupuncturist, and I feel like I have my life back, at least in part. I know that this is going to be a part of my life for now on--when I get tired of taking the time to stretch and build those muscles, I really pay for it in the future. Guest guest Posted June 6, 2009 Report Share Posted June 6, 2009 That's a great point Jane, although I made that suggestion as the vasy majority of PT's out there are BS, and there is no way for an incoming patient to know if the therapist they will see IS so knowledgeable and concerning. Most paeople looking for PT just go to whomever is near their house, but seeking out the best is important, and looking for someone with a MAsters degree is one way to advertise that you are that way. No one advertizes that they are better than the rest, but having an MS tells you that they are highly trained and they care about doing a good job. You can;'t tell the difference between a PT with a BS who is better than others by looking at them or their name. This is why I made the valid suggestion to look for someone with a Masters' degree. OR, do PT in an environment of a physiatrist. Their clinics often have their own PT, who are usually top rate. I think his expertise is why I have been able to walk again after my laminectormy and posterior lumbar fusion on December 29, 2008. I know that I have a lot of time yet to go to get recovered from my surgery and that the pain I deal with daily is just something that is part of life. I just wanted to make the point that talent and professionalism can come with a lesser amount of training. Genuine concern for doing a good job is a major factor as well. I enjoy being a part of this group and certainly read the posts every day. I am a former special education teacher who has resorted to working at home for the past three years due to my increasing spinal stenosis. I have had two anterior cervical fusions,one in 2000 and one in 2008. The worst experience of my life in my 56 years on this planet was the laminectomy and fusion of L4,L5 and S1. This has been a long term stay in hell so far. I think that a laminectomy and fusion of something are probably done to people when they enter hell for sure. This has cost me my credit rating, my career ended due to my deteriorating spine, and life just generally has gone down the tubes due to my spinal garbage. In the middle of it all, my ortho decided I needed the C4,5 and 6 he did last year redone this year. Some HUGE yellow lights went off and I have found another orthopedic surgeon who says that the last thing my profoundly deteriorated arthritic spine needs is more surgery. We are in the process of starting the ESIs again...then the next aspect I suppose will be a pain clinic. Thanks for asking...hope I didnt say too much here. I'm not an MD or a nurse, but common sense tells me not to go along with a surgeon who wants to redo an operation he messed up the first time around. What assurance can you have that he won't screw it up again--maybe a different screw-up? I think ( just a idea) that every case is unique and every surgery is not completely successful. Doctors particularly surgeons, sometimes float the idea of giving someone a surgical solution and combined with hope, good surgical technique, and placebo effect, maybe that will be enough. I fought pain for 3 years and did not do well on the conservative treatment so I went to a surgeon hoping for relief. I did not explore the possiblilty of a bad outcome. I honestly did not have the guts to do so, and was hoping beyond belief that he could exact some magic. Now, with very heavy post surgical pain, I am stuck wondering if I can ever seekl this outcome confidently again. So maybe it is a combination of surgeon's screw-ups, unreleastic expectations by the patient and the reality that some of us fall through the cracks. Life itself can be a painful lesson. Just when you are looking forward to retirement, the back goes. The lesson is to try and glean as much happiness and contentment as possible from the present. Take those small beautiful victories like a flower in the garden or a beautiful day or that 15minutes you can fly fish without pain or a hug and a kiss from your lover. I'm not an MD or a nurse, but common sense tells me not to go along with a surgeon who wants to redo an operation he messed up the first time around. What assurance can you have that he won't screw it up again--maybe a different screw-up? I am in quite a bit of pain since changing doctors and no Loritab having been prescribed by this one but hoping that he is going to have some ideas to get rid of some of this stuff. I just know that after an EMG and CT he said absolutely no way do you need more surgery on your neck. So I loved him instantly! Its effective for chronic low level continuous pain but it does not " get on top of " big breakthrough pain. If ULtram worked that well, it would have replaced more common opiates years ago. Physicians love it because its not classified the same as narcotics. My GP will write Ultram but sends patients to the " pain clinic " for anything stronger. spinal problems From: tpowell1977@... It has been life changing for me, my break through pain has all but disappeared as it lasts 24 hours, there is no low point in coverage. It lasts through the night and until the morning. For those with severe pain, short acting narcotic may be needed for breakthrough pain. But it should take the edge off enough to make the pain manageable...which is key. Docs would prefer to prescribe a short acting narcotic for breakthough pain than for continuous pain. I was referring to my siatica, which for some reason punches through with burning and stabbing and makes my life miserable in the afternoons. Mornings are managed effectively with ultram or ultraset, sometimes even Tylenol arthritis. But stronger meds are needed for the PM. True story though. One has to be disciplined about opiate useage. You absolutely must not take them when the pain is under control with something else because efective relief when you need it is what it is all about. spinal problems From: tpowell1977@... Changing doctors means I have nothing here for this nasty pain. Oh well....life goes on. This much pain is becoming normal for me. Guest guest Posted June 8, 2009 Report Share Posted June 8, 2009 Hey Tracey, I tried the ultram er and it made me sick as a dog. I mean like a stomach virus, had my guts hurting and very sick. I had taken the ultram before but just not the er and it was okay. I've had problems with other er drugs like the diabetes medicine glucophage er. So I must have a problem with whatever it is that makes it extended release. Go figure. I'll not ever take that stuff again tho. I even tried it a second time cause I thought I might have actually gotten a stomach virus but it did the same thing again. Guest guest Posted June 9, 2009 Report Share Posted June 9, 2009 Absolutely . it is because of my judicial use of narcotics that I can get relief from the Ultram now. I didn't get any relief when I was taking Percocet on a regular basis. It wasn't until I droppped the narcs with the exception of break-though pain that I got relief. I am now able to get relief of break-through pain with Tylenol #4...which is amazing! When i do need some, I start with only a half tab...my goal is necver to take the pain away...although that would be lovely...but it's to make it manageable. I have to rely on these meds to work for me for a lifetime...so I'm extra careful. Also, when you do that, you increase your pain tolerance a lot. When you take narcs often, you get used to that level of pain control, so levels of pain which were acceptable before...become difficult to bear. Narcs can be difficult to manage and lead you on a ride if you aren't careful. I know some people scoff when I say Ultram can work on the pain and they have to cut back and or stop the narcs for that to happen...but it's true. They will adjust to the pain levels and the narcs will become more effective. The motivating thought for me was that if I kept going, I was soon going to run out of anything that will work if I didn't stop the process. So, I stopped it before it got to that point.People here say that their pain is just too high that the Ultram doesn't work...and it's NOT that. Believe me, I have had six surgeries and have continued to work, many times in agonizing pain. I worked in so much pain that I developed a heart condition. But now it's better, and the Ultram ER has changed my life. It never wears off! I don't have to stop and take meds all day! So I'm able to forget about my chronic pain and live my life.
