Everything was " normal " (despite feeling terrible, having chills, fatigue, headache, sneezing) except that my IgE jumped from 350 pre Xolair to 1,370 on Xolair. I've read, on this board, I believe, that IgE will jump up on Xolair but that it is not " available " - i.e., somehow is allegedly not affecting the body. Could someone explain this to me again? Guest guest Posted May 1, 2007 Report Share Posted May 1, 2007 Dear SD, It took me 1.5 YEARS to feel better on xolair. IgE values measured post xolair are meaningless. We cannot interpret the usual tests in the same way as the test cannot differentiate between bound and un-bound IgE. Personally, I think its a waste of resources to run IgEs post xolair but that is just IMHO. Recall that xolair is not a miracle treatment despite all the positive reports you may get here. In many people it has helped tremendously, in others it has helped some and in a smaller group, it has made no difference. It has helped me some but I ain't cured yet.... Elevated serum total IgE levels may persist for up to 1 year following discontinuation of XOLAIR. Guest guest Posted May 2, 2007 Report Share Posted May 2, 2007 Just to expand a bit more - The tests currently available to measure IgE cannot tell the difference between plain old IgE and IgE that is bound to Xolair and the Xolair-IgE complexes can accumulate and hang around for a while in the blood. That's why the IgE levels increase over time while you're receiving Xolair. However, the IgE that is bound to Xolair cannot trigger allergic responses by binding to mast cells. That is way the IgE levels measure while on Xolair and for up to a year afterward are not clinically useful, including as a basis for recalculating Xolair dosing. There are ways to measure free IgE without including Xolair-bound IgE but they are not available commercially. I agree completely with that testing for IgE under these conditions is a waste of resources. Do you know why your doctor tested for IgE after you were already on Xolair? Guest guest Posted May 2, 2007 Report Share Posted May 2, 2007 Hi, I'm confused by the doctor choosing to do labs, too. I've had four treatments, and all my doc does is check my breathing beforehand. It improved the first couple of times, but then I had to wait an extra week for a shot, and so my exhales weren't quite as strong this last time. Still, my peak flows are MUCH better than the past few Springs. BTW, the woman who gave me the shots explained that it takes up to a year for the Xolair to build up in your system enough so that you don't feel worse in the days right before your next shot is due. Are you keeping track of your peak flows? Don't give up. The Xolair really might help! Guest guest Posted May 4, 2007 Report Share Posted May 4, 2007 Hi all ~~~~ I don't know why the doc ordered the IgE test either but it was perfectly ok w/me. I have a lot of confidence in this doc - in a major med center. What I don't understand is: It seems like Xolair is sort've the garbage collector running around nabbing all those nasty excessive IgEs in the blood and disarming them someplace with a high wall so that they cannot get out and bind to mast cells and get the allergic cascade of events going. (Obviously these are my words!) So, after 3 series of shots there's a whale of a lot of IgE in me. So, either the Xolair is holding up a lot of IgE or there's an awful lot running around free causing trouble. I can't help but wonder if there's some sort of feedback mechanism going on that's saying " whoa, we don't have all the IgE we used to have so let's make more. " ???? I just don't think the whole story is out there yet. It seems that Xolair should be reducing the amount of IgE and there should be some sort of improvement. I'm not any worse and not any better in my opinion re asthma. BUT, I've had a long term cold and horrible sinusitis for 2-1/2 weeks. I have a slightly uncomfortable feeling that Xolair is contributing to it as Xolair is associated with upper respiratory infections and sinusitis. (I've got a history of sinusitis.) I feel ever so slightly strange. My doc's office said 6 months to see if it's doing any good. But, they also seemed concerned that my IgE went up this high this fast. They're contacting Genentech. My suspicion is that MOST INFO FLOATING AROUND ABOUT XOLAIR IS OUT OF DATE since it's mostly based on the trials. For instance, I now know that my weight is not an issue in determining dosage now - it's the IgE. I had another IgE test today and I know that this level of docs I'm dealing with would not do it w/o a really good reason. My injections are all at home - I mixed and inject myself w/absolutely no problems. I guess I'll know more about this in a few days.
