I used to buy from them years ago. Had not done so in a while but if I recall correctly, I never had any problems with them and always had good service back then.

Would your cousin be interested in an MD? drfurhman.com is in NJ. I don't think he takes insurance however. I can't help with website question.

Yes JOEL Fuhrman. I think he is very pricey according to SAD minded people like my sister who refused to go and so close by, but for me, it would be worth it.
